Position: Registered Nurse (RN) - Geriatrics Extended Care (GEC) Coordinator
The Registered Nurse - Geriatrics Extended Care Coordinator is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates the effectiveness of the Veteran's care. Serves to support delivery of high quality, timely care to the Veteran receiving services in the community. This position is located in Community Care at the Tomah VA Medical Center in Tomah, WI.

The Registered Nurse (RN) - Geriatrics Extended Care (GEC) Coordinator executes position responsibilities that demonstrate expert leadership, experience, and creative approaches in providing complex patient management. They provide patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings.

The RN - GEC Coordinator's focus is on individual growth and development in practice with increased levels of self-direction. This RN brings principles and theory with limited application of critical thinking skills, while assisting Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. These health care needs include, but are not limited to, acute, chronic, multiple, complex, catastrophic, or life-threatening illnesses;

combat stress, residuals of traumatic brain injury; community adjustment; and addictions.

The GEC RN is also responsible for the following duties:

* Coordinating care with multiple providers across all levels and sites of care. Coordination includes assuring services interface within the Veterans Affairs (VA) and Community Care GEC programs to provide continuity of care to patients.

* Possessing and using knowledge of utilization management processes, clinical documentation requirements, current standards of care, and compliance guidelines.

* Maximizing third-party payments for care and delivering quality patient care performance improvement activities.

* Developing and coordinating seamless transitioning of patients throughout the inpatient and outpatient care process; with a focus on discharge planning needs.

* Reviewing and assisting in patient referrals for the following CC GEC programs:
Home Health Aide (HHA), Veterans Directed Care (VDC), Adult Day Health Care (ADHC), In-Home Hospice, In-Home Respite, In-Home Skilled Nursing, Home Infusion, and Contracted Nursing Home (CNH) programs to ensure continuity of qualified comprehensive care.

* Participating, as needed, in the discharge planning assessment and implementation of community health services.

* Performing follow-up and oversight visits to the Veteran after admission as outlined in the Office of Integrated Veterans care (IVC) Field Guidebook; to ensure that adequate and safe care is being provided while under VA contract. Advocates for holistic care through patient/family education and provider interaction and implements an educational plan to meet the patient care needs.

* Collaborating in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines.

* Demonstrating performance and leadership that is broad enough to improve the care for a group of patients.

* Providing peers with formal or informal constructive feedback for improvement.

* Supporting colleagues and other nurses through knowledge sharing to provide safe, quality nursing care.

* Fostering a safe and supportive environment conducive to the professional development of health care professionals.

* Evaluating outcomes of evidence-based decisions and practice changes for individuals, groups, and populations.

* Documenting broad and complex outcomes at the program or service level, that can be demonstrated at any organizational level within a facility.

* Other duties, as assigned.
