The Outpatient/Specialty Care (RN) is responsible for providing competent, evidence-based outpatient care to assigned patients, including oversight of licensed vocational nurses (LVNs)/nursing assistants (NAs), as appropriate to the setting. They assume responsibility for the coordination of care focused on patient transition through the continuum of patient care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction.
The Outpatient/Specialty Care RN is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. The person in this role assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
The Outpatient/Specialty Care RN administers medications and procedures per established policies and guidelines, and influences care outcomes by collaborating with members of the interdisciplinary team.
The Outpatient/Specialty Care RN is also responsible for the following duties:
* Assisting in directing the provision of nursing education, orientation, and competencies.
* Providing quality improvement and outcomes utilization consultation.
* Demonstrating leadership in delivering and improving holistic; providing outpatient care through collaborative strategies with others.
* Providing peers with formal or informal constructive feedback for improvement.
* Supporting colleagues and other nurses through knowledge sharing to provide safe, quality outpatient nursing care.
* Fostering a safe and supportive environment conducive to the professional development of health care professionals.
* Evaluating outcomes of evidence-based decisions and practice changes for individuals, groups, and populations.
* Documenting outcomes at the program or service level.
* Other duties, as assigned.
