BAYADA Home Health Care is seeking a Home Health Physical Therapist to serve adult patients in New Jersey. Positions are available across multiple counties, with flexible full‑time, part‑time, and per diem schedules.
Locations and Compensation Highlights- Atlantic County – Full Time, $10,000 Sign‑on Bonus
- Cape May County – Weekend, Per Diem
- Cumberland County – Full Time
- Essex County – Full Time, $10,000 Sign‑on Bonus for Admissions
- Gloucester County – Full Time
- Hudson County – Full Time, $10,000 Sign‑on Bonus
- Monmouth/Middlesex/Somerset/Union – Full Time, $20,000 Sign‑on Bonus
- Ocean County – Full Time, $25,000 Sign‑on Bonus
- Salem – Per Diem

What You’ll Do- Make home visits to clients in designated geographic territories.
- Perform diagnostic tests and measurements (mobility, range of joints, transfer status, stability, ambulation patterns, strength, endurance, balance, safety assessments).
- Implement integumentary protective interventions to prevent wound formation and promote wound repair.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
- Continually assess and revise the PT care plan; collaborate with nursing and multidisciplinary team.
- Educate and instruct clients, family members, or other representatives on rehabilitative care and activities to promote health, safety, and independent living.
- Accurately document observations, interventions, and evaluations using a state‑of‑the‑art touch‑pad tablet.
- Active and current PT license in New Jersey.
- Previous home health care experience welcomed, but not required – training provided.
- Ability to travel locally to assigned cases.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem‑solving skills.
- Solid computer skills; experience with electronic medical records (EMR) preferred.
- Graduation from an APTA or AAMC‑ACHA accredited program (as shown by transcript or diploma).
