Role Summary

We are seeking a full-time Support Coordinator to join our team. In this role, you will manage a caseload of individuals with developmental disabilities, helping them access and maintain services in their communities. You'll serve as a vital advocate and liaison between individuals, families, providers, and state systems.

Key Responsibilities

• Coordinate and monitor services for individuals with developmental disabilities

• Conduct intake meetings and develop person-centered service plans

• Facilitate the initiation of services with approved state vendors

• Maintain monthly contact and ensure services align with individual needs

• Provide emergency on-call support as needed

• Accurately document and update information in the Consumer Information System (CIS)

Required Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, Counseling, Gerontology, Special Education, or Rehabilitation

• Minimum 1 year of experience working with individuals with intellectual/developmental disabilities

• Proficient in Microsoft Word and general computer applications

• Valid driver's license and proof of auto insurance

Preferred Qualifications

• BOGGS Support Coordination Orientation Certificate
