Hospice Community Liaison
Full-Time | Field-Based | Territory-Focused
Vitality Hospice & Palliative Care is seeking a relationship-driven Healthcare Community Liaison to grow and strengthen our referral partnerships across the local market.
This role is ideal for professionals with experience in senior living, assisted living, skilled nursing, home health, hospice, or healthcare account management who understand the importance of census growth, trusted partnerships, and compassionate patient care.

Position OverviewAs a Hospice Community Liaison, you will manage and grow a defined territory by building strategic referral partnerships with:
- Assisted Living and Independent Living communities
- Skilled Nursing Facilities (SNFs)
- Hospitals and discharge planners
- Physician offices and specialty clinics
You will serve as the face of Vitality Hospice in the community, driving awareness, education, and referral growth while upholding the highest standards of compliance and compassion.
Key Responsibilities Grow & Manage Referral Partnerships- Develop and maintain strong relationships with senior living communities and healthcare partners
- Execute a strategic territory growth plan to increase referrals and census
- Conduct routine in-person visits to assigned accounts
- Identify and cultivate new referral opportunities
- Respond promptly to referral inquiries
- Collaborate with clinical teams to ensure smooth transitions of care
- Maintain HIPAA-compliant communication with case managers and discharge planners
- Deliver educational in-services and presentations
- Coordinate appreciation events and outreach initiatives
- Represent Vitality at networking events and community functions
- Document account activity and referral data in CRM
- Monitor referral trends and adjust engagement strategies accordingly
- Work toward defined growth objectives within the territory
- Experience in senior living sales, home health, hospice, SNF marketing, or healthcare account management
- Proven ability to build and maintain referral relationships
- Strong understanding of census-driven environments
- Comfort with territory management and field-based work
- Excellent communication and follow-up skills
Healthcare sales experience preferred. Existing relationships in the local market are a plus.
