• Participate in a two week clinical orientation
• Assess, evaluate, and develop care plans for patients
• Provide education and care strategies for patients and families
• Collaborate with VNA clinicians and external providers
• Document changes and update care plans

• Active NJ LPN License
• 1+ year of supervised experience (home health or medsurg preferred)
• Excellent communication and problem solving skills
• Car and valid driver’s license

Working indoors and outdoors with clinical equipment and use of phone and computer;

In compliance with New Jersey’s Pay Transparency Act, the compensation range for this position is USD $26.00 to USD $34.00.
