As a Senior Community Care (SCC) team member, you will provide care to patients at home, nursing homes and assisted living facilities for senior housing.
Sign‑On Bonus$2,500 sign‑on bonus for external candidates.
Work ScheduleMonday‑Friday, 8:00 a.m. – 5:00 p.m. EST. No weekends, no holidays, no call requirements.
Role SummaryYou will be part of the primary care team collaborating with all interdisciplinary team members and support multiple providers. The RN works alongside nurse practitioners (NPs) and physician assistants (PAs).
Primary Responsibilities- Collaborate with all members of the interdisciplinary care team regarding changes in condition and care coordination.
- Triage member needs to the appropriate care team member and level of care.
- Assist with transitions in care, both outpatient and inpatient.
- Assist in ensuring complete and accurate documentation, medication reconciliation, nursing rounds, and risk assessments.

- Associate degree or higher in Nursing (RN).
- Active unrestricted Registered Nurse license in your state of residence or ability to obtain within 60 days of hire.
- Certified in Basic Life Support or ability to obtain by start date.
- Driver’s license and reliable transportation to travel up to 100 % to visit clients and/or patient sites within a designated area.
- 1+ years clinical experience as a Registered Nurse.
- BSN degree.
- Long‑term care (LTC) or skilled nursing facility (SNF) experience.
