We are seeking a compassionate and skilled Medical Social Worker (MSW) to join our Home Health & Hospice team based out of Tooele, Utah. This is an opportunity to do meaningful work in the lives of patients and families while collaborating with an interdisciplinary team dedicated to quality care and exceptional service.

What You'll DoAs a Medical Social Worker, you will provide social work services to patients and families receiving home health and hospice care.
Your responsibilities will include:
Providing psychosocial assessment, counseling, support, and social work services to patients and families.
Helping patients and families navigate emotional, social, financial, and community-resource needs.
Supporting patients and families as they make decisions and plan for changing healthcare needs.
Providing compassionate emotional support related to serious illness, end-of-life care, grief, loss, and life transitions.
Connecting patients and families with appropriate community resources and support services.
Participating as an active member of the interdisciplinary team (IDT).Communicating effectively with patients, families, physicians, nurses, therapists, and other members of the care team.
Completing accurate and timely clinical documentation in accordance with applicable regulations and organizational standards.
Supporting individualized care plans that promote dignity, comfort, quality of life, and patient independence.
Maintaining professional standards while managing multiple patients, priorities, and deadlines.
What We're Looking For Master of Social Work (MSW) degree.
Current, unencumbered Utah social work licensure appropriate for the position.
Strong clinical, organizational, assessment, and communication skills.
Ability to work effectively with patients, families, caregivers, and interdisciplinary healthcare professionals.
Ability to manage multiple priorities and meet documentation and scheduling deadlines.
Compassion, professionalism, integrity, and respect for patients and families.
Home health, hospice, medical social work, palliative care, or healthcare experience is strongly preferred.
