Heritage Grove Estates is seeking a Speech Language Pathologist (SLP) to join our in‑house Skilled Nursing Facility (SNF) team in Topeka, Kansas. • Demonstrate a positive attitude and team‑building approach with patients, clients, and team members
• Consistently attend work and provide appropriate notice for scheduled paid time off
• Maintain productivity standards as determined by supervisor and clinical needs
• Demonstrate consistent knowledge and application of Medicare and other reimbursement regulations
• Evaluate patients based on physician referrals and orders
• Develop effective treatment plans with functional, goal‑based outcomes and ensure physician approval/orders for services
• Treat patients according to established treatment plans
• Complete clinical documentation in accordance with accepted regulatory and professional guidelines
• Record daily treatment charges per approved billing guidelines
• Instruct families and nursing staff on restorative and home programs
• Participate in discharge planning
• Communicate with supervisors and healthcare team members regarding patient progress, concerns, and plans of care
• Participate in interdisciplinary team meetings, patient care conferences, utilization review meetings, and other required meetings
• Participate in in‑service training programs for facility staff
• Participate in facility performance improvement programs
• Coordinate care with other departments, attending physicians, committee members, and rehabilitation staff
• Screen patients based on identified needs
• Assist with cleaning and maintenance of treatment areas and department spaces
• Secure necessary durable medical equipment and adaptive equipment for patients
• Report equipment issues to appropriate personnel to ensure proper maintenance and functionality

• Work performed in well‑lighted and ventilated office and therapy areas
• Sitting, standing, bending, lifting, and moving intermittently during working hours
• Regular communication with medical staff, nursing personnel, and department supervisors

• Graduate of an accredited college or university
• Valid state Speech Language Pathology License or eligibility to obtain licensure in the state where services are provided
• Education:
  
  Master’s degree (Preferred)
• License/Certification:
  Speech Language Pathology License (Required)
