Heritage Grove Estates (ALF) is seeking a compassionate and organized Resident Care Coordinator to join our team. As a Resident Care Coordinator, you will play a vital role in ensuring the highest level of care and comfort for our residents. This is a full-time opportunity.

• Coordinate Resident Care: Collaborate with healthcare teams to develop and implement personalized care plans that meet the unique needs of each resident
• Assist with Daily Living
  
  Activities:
  
  Provide assistance with daily living activities such as bathing, dressing, and grooming to support residents' independence
• Monitor Resident Health: Conduct regular wellness checks and report any changes or concerns to the healthcare team
• Maintain Accurate Records: Document resident care plans, progress notes, and vital signs in accordance with company policies and regulations
• Communicate with Families: Keep families informed of their loved ones' progress and any changes or concerns
• Participate in Quality Improvement Initiatives: Contribute to quality improvement initiatives to ensure the highest level of care and comfort for residents

• 0 years of experience in a healthcare or senior living setting
• Ability to lift 25 pounds and assist residents with transfers and mobility
• Strong communication and organizational skills
• Knowledge of senior care principles and practices
• Current CPR/First Aid certification
• Flexibility to work varied shifts, including weekends and holidays
