This position plays a vital role in supporting women and their children throughout their recovery journey s role supports patients from admission through discharge planning and transition. The Recovery Navigator assists with care coordination, provides child supervision during group sessions, and participates in an after-hours on-call rotation.
Essential Job Functions- Serve as a consistent recovery support presence beginning at admission, helping patients feel welcomed and oriented to the program.
- Assist in developing and implementing discharge plans in collaboration with the care team, ensuring continuity of support after patients leave Bridgehouse.
- Support patient access to critical services, including healthcare, peer support, transportation, housing, and community resources.
- Provide safe and nurturing supervision of children during on-site group programming.
- Track and document patient progress, engagement, and appointment follow-through.
- Participate in a rotating on-call schedule (1–2 weeks per month) to respond to patient concerns outside of regular hours.
- Maintain accurate and timely documentation of all support activities.
- Foster a welcoming, safe, and recovery-focused atmosphere for both patients and their children.
- Engage in team meetings, required training, and regular supervision.
- Comply with all CKF policies and procedures and related clinical licensure standards.
- Other duties as assigned.
Based on an 8-hour day
