Job Shift: Day Midland Care Connection

Midland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program.

Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas.

Home Health Program

Provides short-term, skilled medical care to individuals in their homes to support recovery, manage chronic conditions, and help patients remain as independent as possible. Services are delivered by a coordinated team of healthcare professionals and may include nursing care, therapy, and education to help patients and families manage health needs safely  program works closely with physicians and caregivers to create personalized care plans that promote healing, prevent hospital readmissions, and improve overall quality of life.

Occupational

Therapy Intern

Works under the supervision of a licensed occupational therapist to help adults, with varying degrees of disability, remain safe and independent in their homes. The internship usually involves observing and assisting with patient evaluations, helping clients practice daily living activities such as dressing, bathing, cooking, and moving safely around the home, and learning how to recommend adaptive equipment or home modifications that improve safety and function.

The intern may also participate in home safety assessments, caregiver education, and fall-prevention strategies. Works with individuals recovering from strokes, managing chronic illnesses, experiencing weakness, or living with dementia. They also gain experience documenting patient progress, participating in interdisciplinary care meetings, and learning how home health scheduling and Medicare-related documentation work.

A typical day may include reviewing patient charts, traveling to several homes with a supervising therapist, assisting during therapy sessions, and completing documentation afterward. The experience helps interns develop clinical skills as well as independence, communication, organization, and adaptability in real home environments.
