Ability KC is seeking a Program Social Worker to support pediatric and adult medical programs. You will orient participants and guardians to services, complete psychosocial assessments, and liaison with rehabilitation teams to update progress and projected discharge dates.
You will identify community resources, provide education and support, and ensure compliance with mandated reporting guidelines. You may serve as backup for other social workers and participate in in-services.
