Midland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on “meeting individual needs through innovative care solutions,” and we serve people across multiple counties in northeast Kansas.
PACEProgram
The PACE (Program of All-Inclusive Care for the Elderly) program provides comprehensive, coordinated care for older adults with complex medical needs, including those who are no longer able to safely live at home and now reside in a long-term care facility. Through an interdisciplinary team approach, PACE delivers medical care, rehabilitation, medications, transportation, social services, and supportive therapies tailored to each participant’s individual needs.
Participants also have access to PACE center services, including clinical care, meals, and social activities when appropriate. By integrating healthcare and supportive services under one program, PACE enhances quality of life, promotes dignity, and ensures participants receive personalized, compassionate care regardless of where they live.
The PACE Facility APRN provides primary medical care and oversight for Midland Care PACE participants residing in long-term care, skilled nursing and assisted living facilities, serving as the primary clinical liaison between PACE and facility staff. The position is responsible for assessment, treatment, and management of acute and chronic conditions, including advanced care planning, medication management, and coordination of care transitions across all settings.
Active participation in interdisciplinary team meetings, care plan development, quality improvement, utilization review, and polypharmacy programs is required, with accountability for key quality indicators such as avoidable hospitalizations and regulatory compliance. The position partners with the interdisciplinary team to deliver high-quality, participant-centered care while maintaining cost stewardship and compliance with all applicable regulations. This position is supervised as designated on the agency’s organizational chart.
- Graduate of an accredited Nurse Practitioner program
- Current APRN licensure and board certification
- Prescriptive authority in the state of Kansas
- Experience in primary care, geriatrics, or chronic disease management
- Strong assessment, communication, and care coordination skills
- Ability to work collaboratively within an interdisciplinary team
- CPR certification
- Reliable transportation and ability to travel for home/community visits
- Ability to work in clinic, home, and community-based settings
- Previous experience in a PACE program
- Background in home health, hospice, long-term care, or community-based care
- Experience working with older adult populations and complex medical conditions
- Familiarity with electronic health records (EHR)
- Strong participant-centered and preventive care approach
- Ability to work independently and manage changing participant needs
- Experience collaborating with families and caregivers
- Ability to recognize early changes in condition to help prevent hospitalizations
