Job Responsibilities- Ensure all activities align with Elara Caring’s vision and the leadership of the Home Health team.
- Maintain patient and staff privacy and confidentiality in accordance with HIPAA Privacy Final Rule.
- Assess assigned caseload to identify physical, psychosocial, and environmental needs using documentation, clinical records, interdisciplinary reports, and on‑site evaluations.
- Assume primary responsibility for the caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment.
- Collect data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status and incorporate that data into the plan of care.
- Perform initial evaluation and re‑evaluate the patient’s nursing needs during each visit; make follow‑up visits as needed.
- Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and other personnel involved with patient care.
- Revise the plan of care in response to identified patient care issues and notify the Clinical Supervisor, physician, facility, and other team members.
- Current, unrestricted state RN license.
- 1+ year experience in a clinical care setting as a nurse.
- Ability to travel within branch/office coverage area, lift, push, pull, and support up to 50 pounds.
- Dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
- Associate’s Degree or Bachelor’s Degree in Nursing (preferred).
- Previous Home Health or Hospice experience (preferred).
- Previous experience with Home Care Home Base (preferred).
