Job Responsibilities
  • Ensure all activities align with Elara Caring’s vision and the leadership of the Home Health team.
  • Maintain patient and staff privacy and confidentiality in accordance with HIPAA Privacy Final Rule.
  • Assess assigned caseload to identify physical, psychosocial, and environmental needs using documentation, clinical records, interdisciplinary reports, and on‑site evaluations.
  • Assume primary responsibility for the caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment.
  • Collect data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status and incorporate that data into the plan of care.
  • Perform initial evaluation and re‑evaluate the patient’s nursing needs during each visit; make follow‑up visits as needed.
  • Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and other personnel involved with patient care.
  • Revise the plan of care in response to identified patient care issues and notify the Clinical Supervisor, physician, facility, and other team members.
Job Qualifications
  • Current, unrestricted state RN license.
  • 1+ year experience in a clinical care setting as a nurse.
  • Ability to travel within branch/office coverage area, lift, push, pull, and support up to 50 pounds.
  • Dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
  • Associate’s Degree or Bachelor’s Degree in Nursing (preferred).
  • Previous Home Health or Hospice experience (preferred).
  • Previous experience with Home Care Home Base (preferred).
