Key Job Duties and Responsibilities
The Registered Nurse is responsible for providing direct nursing care to residents and supervising the day-to-day nursing activities performed by nursing assistants. Supervision is conducted in accordance with current federal, state, and local standards, guidelines, and regulations that govern the facility, as well as any additional requirements set by the Director of Nursing or Unit Manager, to ensure the highest level of quality care at all times.
The Registered Nurse directs the day-to-day functions of nursing assistants in accordance with the facility’s rules, regulations, and guidelines.
Ensures that all nursing personnel assigned to the RN comply with the written policies and procedures established by the facility.
Plans shift services, programs, and activities in collaboration with assigned nursing staff and support personnel, and makes written and oral reports or recommendations concerning shift activities as required.
Coordinates nursing services with other resident services to maintain the resident’s total care regimen.
Ensures nursing service personnel adhere to their respective job descriptions.
Participates in facility surveys and inspections conducted by authorized government agencies upon request.
Reviews and updates residents’ written discharge plans as required.
Interprets department policies and procedures to personnel, residents, visitors, and government agencies as necessary.
Administers professional services such as catheterization, tube feedings, suction, dressing changes, colostomy care, drainage bags, blood draws, massages, range-of-motion exercises, and end-of-life care as required. Performs lab tests, blood pressure measurements, and other routine monitoring as outlined in residents’ care plans.
Admits, transfers, and discharges residents as needed.
Completes administrative duties including medical forms, reports, evaluations, studies, and charting.
Encourages attending physicians to review treatment plans, record and sign their orders, and progress notes in accordance with established policies.
Receives telephone orders from physicians and transcribes them onto the Physician’s Order Form.
Charts nursing notes in a descriptive manner that reflects the care provided and the resident’s response to that care.
Completes accident/incident reports per facility policy and submits them to the Director of Nursing or Unit Manager.
Records new or changed diet orders and forwards the information to the Dietary Department.
Conducts routine charting duties in accordance with established charting and documentation policies and procedures.
Notifies the Nurse Supervisor of staffing needs if assigned personnel fail to report to work.
Reports absentee call-ins, no-call no-shows to the Director of Nursing and/or Scheduling Coordinator.
Evaluates the department’s workforce and makes staffing recommendations to the Nurse Supervisor.
Develops work assignments and assists in completing them as needed.
Conducts rounds of the unit/shift to ensure staff perform assigned tasks as expected.
Builds and maintains positive relationships with co‑workers and other departments within the facility.
Reviews complaints and grievances and reports them to the Director of Nursing and/or Unit Manager.
Provides and receives nurse shift reports when starting and ending each shift.
Reports occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with facility policies.
Monitors assigned personnel to ensure compliance with safety regulations concerning equipment and supplies.
Recommends equipment and supply needs of the unit/shift to the Nurse Supervisor.
Reviews care plans daily to confirm appropriate care is being delivered.
Maintains confidentiality of all resident care information.
