RN; Registered Nurse Case Manager, Hospice

Position

• Full Time:
  Monday - Friday, Days
• Topeka, KS and surrounding areas
• Pay Range: $75,000 - $90,000

Phoenix Hospice RN Case Managers provide compassionate nursing care to clients and their families dealing with a terminal diagnosis. The role works with the interdisciplinary team to develop and implement a patient/family centered plan of care, including goals of care discussions and guidance with healthcare decisions to enhance quality of life.

• Provide education to patients/family members regarding the process, plan of care, and offers guidance with healthcare decisions.
• Consult with the attending physician and Hospice Medical Director concerning changes in patients condition requiring order changes and/or changes to plan of care.
• Coordinate hospice care by working with other team members as well as with the patients/family members to ensure needs are met.
• Conduct initial as well as on-going comprehensive assessments.
• Perform evaluations for hospice care along with informational visits.

• Kansas or Compact RN License
• Valid driver's license and auto liability insurance
