Job Summary

Part‑time Nurse Practitioner – In‑Home Health Evaluations at Signify Health, part of CVS Health®. The role delivers 45‑minute personalized healthcare visits in patients’ homes, utilizing an iPad and a streamlined clinical workflow. The clinician focuses on assessment, medication & medical history review, and diagnostic testing as ordered by the patient’s health plan. Prescribing and lab ordering are not required.

• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
• Be flexible to travel locally and within licensed states, as needed.
• Conduct virtual visits as required.
• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, diabetic eye exams).
• Use an iPad and an intuitive clinical workflow for each evaluation.
• Perform medical history review, physical evaluation, and optional diagnostic tests provided by the health plan.

• Active, unrestricted medical license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).
• Anticipated weekly hours: 29.5.
• Part‑time employment.

Typical pay range: $37.66 - $81.11 per hour. Base salary will be determined based on experience, education, geography, and other factors.
