Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Hospice

& Palliative Care

Midland Care Connection’s Hospice and Palliative Care program is dedicated to providing compassionate, holistic care to individuals living with serious or life-limiting illnesses. Our team focuses on comfort, dignity, symptom management, and quality of life while supporting patients and their families through every step of the journey. Through an interdisciplinary approach, Midland Care provides personalized medical, emotional, spiritual, and practical support in the setting that best meets each patient’s needs.

Nurse

Practitioner

The Nurse Practitioner provides advanced clinical care to patients with serious or life-limiting illnesses, with an emphasis on comfort, symptom management, and quality of life.

The Nurse Practitioner evaluates patients, diagnoses and treats medical conditions, manages complex symptoms, prescribes medications, and develops or updates individualized care plans. They work closely with patients and families to understand goals of care, explain treatment options, and provide guidance throughout the hospice or palliative care journey. The Nurse Practitioner also collaborates with physicians and the interdisciplinary team—including nurses, social workers, chaplains, and other care professionals—to ensure care is coordinated and aligned with each patient’s wishes.

Qualifications

Required
  • Current, unrestricted Nurse Practitioner/APRN license in the state of Kansas
  • Current national board certification as a Nurse Practitioner through an approved certifying body
  • Graduate degree from an accredited Nurse Practitioner program
  • Current DEA registration and ability to prescribe medications
  • Current BLS certification
  • Experience providing direct patient care as a Nurse Practitioner
  • Strong assessment, clinical decision-making, and communication skills
  • Ability to work collaboratively as part of an interdisciplinary care team
  • Ability to provide compassionate, patient-centered care to individuals with serious or life-limiting illnesses
  • Ability to work independently and manage multiple patient needs while maintaining accurate clinical documentation
Preferred
  • Experience in hospice, palliative care, geriatrics, oncology, or another setting caring for patients with serious or life-limiting illnesses
  • Hospice and Palliative Medicine (HPM) certification or willingness to obtain it
  • Experience with symptom management, pain management, and end-of-life care
  • Experience discussing goals of care and advance care planning with patients and families
  • Experience working within an interdisciplinary team
  • Familiarity with Medicare hospice regulations and hospice eligibility requirements
  • Experience providing care in patients’ homes, long-term care facilities, or other community-based settings
