We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our Assisted Living team.

The LPN/RN Nurse provides nursing support and oversight for residents of The Terraces while promoting independence, dignity, choice, safety, and quality of life.
The Nurse is responsible for monitoring resident health and wellness, identifying and communicating changes in condition, supporting medication management, coordinating with providers and families, completing required documentation, and providing clinical guidance to direct-care team members.
This position works closely with facility leadership, Medication Aides, CNAs, providers, residents, families, and other members of the care team to ensure resident needs are appropriately addressed.
Key Responsibilities Resident Care & WellnessMonitor the health, safety, and overall wellness of residents
Complete nursing assessments and evaluations as assigned
Observe residents for changes in physical, cognitive, behavioral, or emotional condition
Obtain and monitor vital signs and other clinical information as appropriate
Provide nursing care and treatments within applicable scope of practice
Support residents in maintaining the highest practical level of independence
Respond appropriately to resident emergencies and urgent clinical concerns
Administer medications and treatments within applicable nursing scope of practice
Monitor medication administration and resident response to medications
Support safe medication-management practices
Provide guidance to Medication Aides within applicable scope and facility procedures
Ensure medication-related documentation is accurate and completed timely
Communicate medication concerns to providers and leadership as appropriate
Recognize and respond to changes in resident condition
Communicate significant changes promptly to the appropriate provider
Coordinate with families or responsible parties as appropriate
Review new orders and ensure appropriate communication and implementation
Help ensure continuity of care between residents, providers, families, and facility staff
Participate in resident assessments and individualized service planning
Help ensure services reflect resident needs, preferences, abilities, and identified risks
Complete nursing documentation accurately and within established time frames
Document assessments, changes in condition, provider communication, treatments, and other nursing interventions
Support appropriate documentation by direct-care team members
Serve as a clinical resource for CNAs, Medication Aides, and other care team members
Provide direction and delegation within applicable scope of practice and facility procedures
Assist staff with resident-care questions and clinical concerns
Promote accountability and timely communication of resident changes
Support new employee orientation and ongoing staff education as assigned
