Seeking a Registered Nurse (RN) for Home Health services in Topeka, KS. Role & Responsibilities

• Provide skilled nursing visits, including assessments, care planning, and patient monitoring.
• Develop, review, and revise individualized care plans per physician instructions.
• Administer treatments, observe patient responses, and report changes to healthcare team.
• Educate patients and families on care procedures and health management.
• Document care and patient conditions through electronic charting, supported by AI tools to enhance efficiency.
• Collaborate with multidisciplinary teams such as therapists and other healthcare professionals.

• Current RN license in Kansas; LPN license acceptable with relevant experience.
• Preferred experience in Home Health or Critical Care.
• Valid driver’s license, vehicle, and current auto insurance.
• Ability to pass background checks, drug screening, and physical assessments.

• Strong physical assessment, diagnostic, and communication skills.
• Ability to work independently while collaborating with healthcare teams.
