Overview

The Case Manager is responsible for coordinating comprehensive services for clients participating in Family Self-Sufficiency programs, with the ultimate goal of helping individuals and families achieve long-term self-sufficiency. This includes providing rental and utility assistance and helping clients build connections with workforce development, financial education, life skills training, and other supportive services based on client assessments. The Case Manager provides referrals and/or connects clients with internal programs and community partners and assists with public benefits applications through the Texas Health and Human Services Commission to ensure clients’ basic needs such as food are met.

The position also involves monitoring daily progress toward program outcomes and reporting requirements, ensuring program compliance and performance.

Responsibilities
  • Assist with day-to-day program operations related to case management services, including assessing and determining eligibility for emergency assistance, requesting and applying for emergency assistance, administering emergency funding, and processing payments for emergency assistance.
  • Ensure all forms are fully completed, all necessary documentation is obtained, and data is entered into appropriate databases (Charity Tracker, Briteworks, Smartsheets, HMIS) in a timely manner.
  • Conduct intake assessments of clients to determine educational, workforce development, financial literacy, and basic needs; provide referrals to additional resources; maintain an active caseload with goals, assessment and tracking of outcomes on a self-sufficiency matrix, and follow up on progress at regular intervals (at least every 30, 60, and 90 days, plus aftercare).
  • Maintain detailed, up-to-date, accurate, and well-written documentation of client progress toward financial, workforce, and personal goals.
  • Submit monthly and quarterly reports in a timely manner to the Associate Senior Director.
  • Provide clients with assistance in navigating and applying for resources such as SNAP, TANF, housing, childcare assistance, or other applicable resources/eligible services with other agencies.
  • Conduct Family Self-Sufficiency program classes to assist in progressing clients toward identified goals.
  • Conduct file reviews to ensure client records comply with agency and funder requirements.
  • Develop and nurture partnerships with external stakeholders to increase agency capacity to deliver adult educational services, workforce development, and financial literacy.
  • Maintain good relationships with community partners and resources.
  • May be required to attend meetings, visit clients, conduct inspections on habitability and lead standards, deliver vendor payments to properties, or pick up items or donations for the program.
  • May be required to conduct outreach to promote services offered.
  • Complete all professional development training in a timely manner.
  • Be sensitive to the service population’s cultural and socioeconomic characteristics.
  • Adhere to safety training and protocols daily and take precautions to ensure the safety and well-being of self and others.
  • Protect the confidentiality of information or material obtained in the service with the organization, including client names and information, services rendered, donors and gifts, investigations, and financial information.
  • Adhere to the Code of Conduct and Faith and Moral values as mandatory.
  • Maintain a solution-oriented approach, promote collaboration, and align with the organization’s Mission and Vision.

Minimum Qualifications
  • Education
    :
    Bachelor’s degree in education, administration, and/or social work. Social work or business-related field.
  • Experience
    : A minimum of 2 years of experience in a non-profit setting, with at least 1 year in social services and/or case management preferred. Bilingual in English/Spanish preferred.
