Job Summary

The Center for Restorative Education (CRE) Case Manager will work closely with the CRE Supervisor to enroll referred participants into the CRE program. The CRE Case Manager will provide assessments, support planning/connection, and delivery of services to youth in the program. The CRE Case Manager will work closely with other program staff and the CRE Supervisor to support youth by utilizing a variety of program‑specific interventions.

The CRE Case Manager will work closely with parents/guardians and other professionals in the community, including school personnel and community-based service providers, to ensure each youth’s goals are obtained. The CRE Case Manager will demonstrate clear and professional boundaries with youth in all communications and involvement they have with the youth and their family.

Essential Job Functions
  • Assessment, Evaluation, Service Planning & Delivery
    • Conducts comprehensive youth assessments using approved, research‑based tools to identify strengths and needs.
    • Develops individualized support plans collaboratively with youth and parents/guardians, ensuring goals, tasks, and timelines are clearly defined.
    • Implements support plans in coordination with families, school staff, and community professionals, following agency protocols and timelines.
  • Crisis Intervention
    • Facilitates small‑group life skills sessions to promote youth well‑being and teach essential coping and decision‑making skills.
    • Provides on‑site crisis intervention during program hours, ensuring immediate, appropriate responses to youth in distress.
    • Maintains strong knowledge of community crisis services, substance misuse resources, and communicates service needs to families and professionals.
  • Communication
    • Builds trusting, professional relationships with youth, families, school personnel, healthcare providers, and community partners through clear, transparent communication.
    • Facilitates communication among program staff regarding student progress, behaviors, and support plan goals; reports updates to the CRE Supervisor.
    • Prepares professional reports for the Court and agency leadership, documenting academic progress, safety concerns, and well‑being.
Education & Experience

Required

  • Bachelor's Degree in Social Work, Human Service, or related field
  • 2 years' experience providing case management or similar work with at risk youth and families.
