Providence Living Center in Topeka, KS is hiring a Licensed Social Worker to join our team.

Position SummaryAs a Social Worker, you will serve as an advocate for residents and their families, working closely with them to ensure their physical, emotional, and social needs are met. You will document residents’ charts, conduct assessments, coordinate care aspects, and promote a positive and supportive environment.

- Discharge Planning
- Collaboration with multidisciplinary team
- Licensed Social Worker
- Experience in long‑term care and memory care preferred
- Ability to work with multidisciplinary team
- Excellent written and verbal communication skills
- HMO, Medicare and insurance experience preferred
- Knowledge of community resources and HIPAA compliance
Full‑time position, 8‑hour shifts, 8am‑5pm with reasonable flexibility.
