Job Summary

We are seeking a Project Manager to support and coordinate the development, implementation, and evaluation of our Neighbourhood Care Team which is an integrated geriatric outreach model that brings multiple service providers to work as one team in support of seniors residing in seniors housing and naturally occurring retirement communities to promote healthy aging  project manager will be responsible for organizing the work of the teams on the ground, work with the tenant communities to develop tenant led activities, and support the model quality improvement, evaluation and expansion.

In addition to that the Project Manager will support targeted quality improvement projects to advance our outpatient specialized geriatric and primary care programs to improve care pathways and patients experience. This full-time role reports to the Clinical Manager, of the Outpatient Specialized Geriatric and Primary Care Services and is critical role to advancing Baycrest Hospital’s strategy to expand care beyond its campus.

Key Responsibilities
  • Coordinate the work of the clinical teams in Community locations including planning, implementation, monitoring, and evaluation.
  • Develop and manage projects’ documentation and tools (e.g., work plans, timelines, communication plans, change logs, reports, and presentations) to support effective execution and accountability.
  • Lead the coordination and facilitation of meetings, working groups, and stakeholder engagement activities to ensure alignment with project goals.
  • Support the design, implementation, and continuous improvement of services, with a focus on capacity building in older adult care.
  • Collaborate with internal and external partners to ensure program relevance, quality, and responsiveness to population needs across the continuum of care.
  • Lead and support program evaluation and reporting.
  • Help develop and implement care pathways.
  • Contribute to quality improvement efforts by embedding best practices and feedback loops into program delivery.
  • Act as a liaison and point of contact for neighbourhood care team locations and teams within designated locations.
  • Serve as a community connector for tenants to facilitate connection to services and support the development of tenant led activities and circles.
