Outreach Community Ambassador

Position: Outreach Community Ambassador Role
Be a vital part of the North Toronto community with a role as a Community Ambassador at Sunnybrook Health Sciences Centre. Engage residents, promote primary care awareness, and facilitate health navigation support.

In this position, you will work closely with the North Toronto Ontario Health Team, actively participating in community outreach efforts.

Your role will include implementing effective outreach strategies and fostering strong community connections. By engaging with residents, you will provide guidance on health resources and support their navigation through health care services.

Key Responsibilities:

• Collaborate on outreach strategies with community health teams

• Execute grassroots outreach initiatives in North Toronto

• Facilitate connections between community members and primary care

• Collect outreach data and assist in project evaluations

• Support health promotion sessions and community events

Requirements:

• Familiarity with the Davisville area and local networks

• Experience in community-facing roles or health promotion

• Strong facilitation and communication abilities

• Willingness to work flexible hours, including evenings

• Self-motivated with strong organizational skills
