Become the Provincial Hep C Program Coordinator with PASAN, where your work will enhance support for individuals at risk of HIV/Hep C within the incarceration system. This role is crucial for advocating education and resources in the community.
As the Coordinator, your responsibilities will include fostering partnerships, managing programs, and advocating for systemic changes affecting prisoners. Utilizing your skills in harm reduction and community health, you will work directly with healthcare providers to streamline access to vital services. Your ability to communicate effectively and work collaboratively will drive program success.
Key Responsibilities:
• Coordinate targeted education on HIV and Hep C topics
• Deliver support and advocacy for those affected
• Build relationships with community organizations for referrals
• Develop work plans to assess program impacts
• Maintain comprehensive documentation for program evaluation
Requirements:
• Experience in harm reduction, community health, and case management
• Strong commitment to diversity and inclusion initiatives
• Excellent verbal and written communication skills
• Microsoft Office proficiency required
• Class G Driver’s License and clearance for prison access
