Program Overview – Community Care, Seniors Active Living Centres & Outreach
Wood Green Community Services’ Outreach to Diverse Seniors program funds grassroots groups and communities to deliver programs for seniors to help them reach their health goals, and enable them to age in community. Grassroots groups deliver programs focused on health promotion, outreach, and connection to existing services. The goal of this program is to provide funding and oversight to grassroots groups to address gaps in health service provision and work towards equity in accessing health services in a community setting.

The Coordinator works collaboratively across Wood Green programs, particularly with the Outreach Team and other Community Care staff, and maintains strong working relationships with external stakeholders, including grassroots organizations, seniors' groups, healthcare providers, hospitals, primary care teams, community health centres, social service agencies, volunteers, and government-funded partners. The role is responsible for fostering collaborative partnerships, promoting equitable access to health promotion initiatives, supporting community capacity building, monitoring program performance, and contributing to data collection, reporting, and continuous program improvement.

What You Will Do

Engage and support grassroots groups who are funded to deliver services for vulnerable seniors in the east and mid-east Toronto sub regions, including project oversight.

Create training curriculum covering topics of community health, health promotion, health advocacy, patient engagement, goal setting, and other topics related to community health.

Develop training programs for community group coordinators in topic areas such as partnership building and system navigation.

Establish positive collaborative relationships with appropriate community agencies, including attending and speaking at community meetings and other functions.

Assist with planning and development of program activities for grassroots communities.

Liaise with staff from other programs, grassroots groups, volunteers, and community agencies (e.g., hospitals, social workers, doctors, health clinics) to provide program information.

Support members of each group to contribute their full potential, develop leadership skills, engage their community, and build long term community capacity.

Work closely with the Outreach Team to review and monitor progress reports and data.

Outreach to new community groups and develop a pipeline of groups for future funding.
