Job Opportunity – Case Manager / Discharge Planning – California

Start Date: ASAP
Location: Torrance, California
Compensation: Details provided to qualified applicants
Schedule: Days, 1:20 patient ratio, every other weekend, holiday rotation required

Highlights:

• Acute care case management and discharge planning in a dynamic hospital setting
• Caseload up to 25 patients with comprehensive interdisciplinary support
• Epic charting system; 40-hour orientation provided
• Work with diverse patient populations across all age groups from neonates to geriatrics
• Inter Qual criteria experience highly valued; focus on care coordination and admission reviews
• Up to 7 days of approved time off available

Requirements:

• Active California RN license (or pending, but must be licensed by start date)
• Minimum 2 years acute care case management or discharge planning experience
• Epic charting system proficiency required
• Strong knowledge of Medicare and Medi Cal guidelines, HMOs, and IPAs
• Experience with Inter Qual criteria, admission reviews, and medical necessity determination
• Competency in arranging DME, SNF placement, and ambulance transport independently
• Ability to assess patients, coordinate care, and lead family meetings
• Experience with complex populations including mental health, substance abuse, homelessness, and protective services
• Available for every other weekend and holiday rotation (must commit to 3 of 5 major holidays)
• First-time travelers accepted with proper screening
