RN - Case Management

Day Shift: 5x8-hour (08:00 - 16:30) with weekend requirements every other weekend.

The Acute Care Case Manager will be responsible for overseeing patient care coordination and discharge planning. This role involves managing a case load of up to 25 patients and requires the ability to collaborate with various interdisciplinary teams to ensure continuity and quality of care within an acute care setting.

• Coordinate services to home care and equipment needs following discharge.
• Assist patients with navigating the Department of Children and Families (DCF) process.
• Document pertinent information to enhance care coordination.
• Participate in care conference activities and coordinate multi-disciplinary team meetings.

• Minimum of 2 years of acute hospital case management experience required.
• California state license required; pending licenses accepted.
• Experience with Epic charting system is required.
• Must commit to working 3 of the 5 major holidays.

  Minimum of 2 supervisory references required.
