Job Summary:

The Registered Nurse provides skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. Responsibilities include patient assessments, medication administration, treatment planning, and collaboration with an interdisciplinary team to ensure optimal patient outcomes. Strong communication and collaboration skills are essential for success in this role.

Responsibilities:

- Conduct patient assessments to determine care needs.
- Administer medications and treatments as prescribed.
- Develop and implement patient care plans.
- Collaborate with the interdisciplinary team to coordinate patient care.
- Provide skilled nursing care in various settings (e.g., Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, Operating Room, home, hospice).
- Document patient care accurately and timely.

Required Skills &

Certifications:

- Active Registered Nurse license in the state of working.
- Bachelor's degree in Nursing or related field.
- Minimum of 2 years of hospital, home health, or hospice care experience.
- Clinical Assessment skills.
- Medication Administration skills.
- Treatment Planning skills.
- Interdisciplinary Team Collaboration skills.
- Driver's license, car insurance, and automobile for field work (if applicable).
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.

- Speciality certifications in relevant areas (e.g., Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, Operating Room).
