Job Summary:

The Registered Nurse is responsible for providing skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. This role involves patient assessments, medication administration, treatment planning, and collaboration with an interdisciplinary team. Strong communication and collaboration with patients, providers, and team members are essential.

Responsibilities:

- Providing skilled nursing care to patients.
- Performing patient assessments and monitoring their conditions.
- Administering medications and treatments as prescribed.
- Developing and implementing patient care plans.
- Collaborating with the interdisciplinary team to ensure comprehensive patient care.
- Providing case management services.
- Delivering hospice care to patients in need.
- Maintaining accurate and up-to-date patient records.
- Communicating effectively with patients, families, and healthcare professionals.

Required Skills &

- Active Registered Nurse license in the state of working
- Bachelor's degree in Nursing or related field
- Driver's license, car insurance, and automobile for field work
- Minimum of 2 years of hospital, home health, or hospice care experience
