Registered Nurse Case Manager

Job Summary:

The Registered Nurse Case Manager is responsible for providing skilled nursing care, case management, and discharge planning within an acute care facility. This role involves patient assessments, treatment planning, collaboration with the interdisciplinary team, and ensuring effective communication with patients and providers. The position requires at least two years of acute care case management/discharge planning experience.

Responsibilities:

- Provide skilled nursing care and case management services.
- Conduct patient assessments and develop treatment plans.
- Collaborate with the interdisciplinary team to ensure coordinated care.
- Facilitate discharge planning for patients.
- Administer medications and perform therapeutic procedures.
- Maintain accurate and timely documentation of patient care.
- Communicate effectively with patients, families, and healthcare providers.

Required Skills &

Certifications:

- Active Registered Nurse license in the state of California.
- Minimum of 2 years of acute care case management/discharge planning experience.
- Clinical Assessment, Medication Administration, and Treatment Planning skills.
- Interdisciplinary Team Collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.

Preferred Skills &

Certifications:

- Bachelor's degree in Nursing or related field.

Special Considerations:

- Must be available to work every other weekend and holidays.
- RTO restrictions:
No more than 7 days.
- First-time travelers are accepted but must be screened.
- No floating expectation.

Scheduling:
- Day shift with a 1:20 patient ratio.
