Job Description
HOME HEALTH RN - HOME HEALTH

Schedule
- Start Time: 08:00

Requirements
- Active California nursing license
- 2 years of home health experience with high-volume caseloads
- Recent OASIS experience required
- Previous home health travel experience required
- 2 supervisor references from the past 12 months
- Valid driver's license
- BLS certification
- Flu vaccine required

Roles & Responsibilities
- Provide skilled nursing care to adult and geriatric patients in their homes.
- Conduct assessments and develop patient care plans focused on heart failure management.
- Perform wound care, PICC/central line dressing changes, and IV therapy.
- Document care and updates using OASIS standards during visits.
- Administer medications and manage anticoagulant therapy as needed.
- Ensure compliance with infection control protocols and best practices in care.

Compliance / Notes
- Weekend work required (2 weekend shifts per contract references).
- On-call duties required.
- No time off within the first 2 weeks; total RTO should not exceed 10 days.
- Background checks conducted; DUIs may be considered depending on recency.
