Premier Infusion & Healthcare Services is a preferred post‑acute care partner delivering high‑quality home health and infusion services in Southern California. The Licensed Practical/Vocational Nurse (LVN) provides direct patient care under the supervision of a registered nurse.
Job SummaryThe LVN offers direct patient care, follows the plan of care, documents accurately, educates patients/families, coordinates services, and collaborates with the interdisciplinary team.
Responsibilities- Provides direct patient care as defined in the state Nurse Practice Act.
- Implements the plan of care initiated by the registered nurse.
- Documents accurately and timely in accordance with the plan of care.
- Assesses and educates patients and family/caregivers on self‑care techniques and diagnosis information.
- Coordinates home health services and reports needs to the registered nurse and/or Clinical Supervisor.
- Uses and prepares equipment and supplies while adhering to aseptic technique.
- Participates in personal and professional growth and development.
- Performs duties as assigned by the registered nurse, including inspections, performance improvement programs, committees, education programs, and reporting misconduct.
- Supervises home health aides.

- Problem‑solving and basic math skills.
- Certification in nursing competencies per state requirements.
- Physical ability: prolonged walking/standing, lifting, transferring patients, moving supplies, extensive reaching, stooping, bending, kneeling, crouching, and adequate visual acuity and hearing.
- Graduate of an accredited practical nurse or vocational nursing program.
- IV/BW (Intravenous/Blood Withdrawal) certification from a CA Board‑approved provider.
- Minimum of two (2) years of nursing experience, with at least one (1) year in home health.
- Currently licensed as a licensed practical nurse or licensed vocational nurse in California.
- Valid CPR certification.
$30.00 – $45.00 per hour
