We have immediate opportunity for experienced Advanced Practice Clinician to join our High Risk team in Torrance, CA. The clinicians we seek are those who practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient's needs.

Primary Responsibilities:

• Optum Clinic Locations:
  Torrance, CA
• Home visits with high acuity patient population including palliative care
• Average Patients Per Day 4-5 home visits with 2-3 telehealth session
• Patient Mix:
  Adult patients with advanced cancers and patients >65 with end-stage chronic medical conditions e.g., CHF, COPD or may be bedbound
• No weekend or after hours clinics
• Primary area of responsibility for this position is the Torrance, CA area and the surrounding communities
• Visits are scheduled 2 days in advance

Required Qualifications:

• Master's degree from a four-year college and/or a professional certification beyond a four-year college
• Certificate of completion from an accredited Nurse Practitioner or Physician Assistant Program
• Unrestricted California Advanced Practice Clinician license
• Current AANP/ANCC/NCCPA certification
• Current California DEA certificate required prior to start date
• 2+years of experience in ambulatory care or a similar environment
• EMR Proficient
• Excellent patient care and time management skills
• Ability to work in fast pace environment, team player, adaptable to changing priorities
• Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. Preferred Qualifications:

• Strong Geriatric or Palliative care background
• Previous experience in a managed care setting and working with medically complex patients
• Medical Spanish preferred but not required
