The Western Connecticut Mental Health Network (WCMHN), under the umbrella of the State of Connecticut, Department of Mental Health and Addiction Services (DMHAS), is seeking to hire a part-time Advanced Clinical Social Worker within the Access Team at the Western CT Mental Health Network - Torrington Area (WCMHN-TA).

THE ROLE

WCMHN-TA's Access Center provides Mobile Crisis Services to the 19 towns in the CAC 22 service area, supports numerous contract agencies serving that area, and coordinates care with the local hospital. The clinician functions as a member of an interdisciplinary Mobile Crisis Team that provides crisis evaluations and interventions in the community.

Key Responsibilities
  • Clinically assesses risk in the community to formulate immediate interventions and administration of Emergency Certificates as clinically indicated
  • Provides services via the Regional Hotline with the skills to access/triage/activate resources for people who request information, support, or crisis intervention
  • Provides intake services to the 19 towns in the CAC 22 area by completing a psychosocial assessment, risk assessment, and other admission paperwork for the purpose of admitting clients to the agency
  • Communicates and works collaboratively with contract agencies, community providers, State of CT DMHAS and community hospitals as well as local and State police
  • Links community members to appropriate care, provides community resource information, and manages risk for negative outcomes in the community
  • Provides coverage for the Jail Diversion program as assigned by the program supervisor
  • Completes assessments and makes clinical recommendations for treatment as requested by the court
  • Coordinates care and monitors participation with psychiatric and substance abuse treatment services
  • Provides individual and group psychotherapy to a small caseload
  • Develops and implements person-centered recovery plans to guide treatment
  • Participates in case conferences and critical incident reviews
  • Documents treatment interventions in the electronic health record and completes reports and screenings in accordance with agency policies and procedures
  • Participates in organizational committees and trainings as assigned
  • Provides shift oversight for the Access Center staff as assigned by the program supervisor
  • Provides clinical supervision to assigned Access Center staff and to students and/or interns as assigned by the program supervisor
