Job Summary

Advanced Clinical Social Worker – Access Team (Mobile Crisis) at Western Connecticut Mental Health Network (WCMHN), Torrington, CT. Full‑time, 40 hours/week, Tuesday‑Saturday, 8:00 a.m.–4:30 p.m.

Responsibilities
  • Conduct community‑based crisis evaluations, assess risk, and formulate immediate interventions—including completing Emergency Certificates when indicated.
  • Provide crisis triage, support, and resource activation through the Regional Hotline.
  • Complete intake services for the 19 towns in the CAC 22 area, including psychosocial and risk assessments and all required admission documentation.
  • Collaborate closely with contract agencies, community providers, DMHAS, local hospitals, and police to coordinate care and ensure client safety.
  • Link individuals to appropriate services, offer community resource information, and manage risk to prevent negative outcomes.
  • Provide coverage for the Jail Diversion program, including court‑requested assessments and clinical recommendations for treatment.
  • Coordinate care and monitor client engagement in psychiatric and substance‑use treatment services.
  • Deliver individual and group psychotherapy to a small caseload.
  • Develop and implement person‑centered recovery plans that guide treatment and support recovery goals.
  • Participate in case conferences and critical incident reviews to support continuous quality improvement.
  • Document all interventions, assessments, and required reports in the electronic health record in accordance with agency standards.
  • Serve on organizational committees and participate in staff trainings as assigned.
  • Provide shift oversight for Access Center staff and offer clinical supervision to assigned staff, students, and interns.
Agency & Facilities

The State of Connecticut, Department of Mental Health and Addiction Services (DMHAS), provides integrated, holistic behavioral health services. WCMHN, formed in 1996, coordinates clinical, fiscal, and administrative oversight of Region 5 services. Over 200 staff—including psychiatrists, nurses, social workers, and allied professionals—offer comprehensive care for individuals with long‑term mental illness.

Minimum Qualifications
  • One (1) year of experience as a licensed clinical social worker.
  • License to practice clinical social work pursuant to Sec. 20‑195 (m‑z) of the Connecticut General Statutes.
Preferred Qualifications
  • Experience performing intake (psychosocial) assessments and initial care planning.
  • Experience completing community‑based crisis assessments.
  • Experience completing risk assessments and developing risk mitigation plans.
  • Experience providing trauma‑informed services to individuals with severe and prolonged mental illness, addiction, and co‑morbidity.
Special Requirements
  • Must possess and retain a license to practice clinical social work as required by state law.
  • Must maintain eligibility for participation in federal health care programs.
  • Knowledge of sign language and communication ability with deaf and hearing‑impaired individuals may be required.
  • Ability in Spanish oral and written communication may be required in designated positions.
  • Travel may be required.
