Position: HERO Peer Case Manager

Overview

HERO Peer Case Manager

Works as an integral member of the HERO (Housing Empowering Recovery Opportunities) team to assist clients who are homeless or at risk of homelessness, have a substance use disorder, and are at risk of overdose to obtain a housing subsidy and provide case management services to support their recovery. Has responsibility for understanding and endorsing multiple pathways to recovery

Full-time / Non-Exempt

Pay Range begins at $24.00

MINIMUM QUALIFICATIONS

2 years’ experience working with homeless population and/or lived experience required. Candidates with an Associate's degree plus a minimum of 3 years’ experience will be considered. Must have excellent verbal and written communication skills and knowledge of community resources. Peer support certification required within 90 days of hire.

Knowledge of, or ability to quickly develop competency with, issues related to homelessness, effective intervention techniques, and basic understanding of mental health and substance abuse issues.

Must be able to work independently and in the community, maintain good working relationships with organizational staff and partners, and represent the values and philosophy of McCall in all interactions.

KEY RESPONSIBILITIES
  • Assist clients in completing and submitting the DMHAS housing rental assistance subsidy application.
  • Provide housing coordination as needed.
  • Provide liaison services between tenants, landlords, and other service providers.
  • Provide employment services or referrals.
  • Provide advocacy for equal access to housing services.
  • Assist clients in completing the HERO participant agreement and identifying service and support needs.
  • Complete HERO Case Management assessments as assigned, including at least monthly in-home, face-to-face case management.
  • Develop and implement a recovery plan with each client.
  • Assist clients in accessing services and developing self-advocacy and natural supports.
  • Participate in HERO meetings.
  • Implement overdose prevention strategies and resources
  • Any other duties assigned.
