LICENSED PRACTICAL NURSE - ELIGIBLE FOR SIGN-ON BONUS of $3,000
The Licensed Practical Nurse serves as part of a multidisciplinary team. The role aids the medical providers in monitoring clients needs and coordinating care. The LPN will also provide medication education and will administer injectable medications when needed.
Non-Exempt, Full Time
Pay range begins at $27.00 / hour
QUALIFICATIONSDegree/certification in nursing or in a related health care field; training as a licensed practical nurse; prior knowledge of Medication-Assisted Treatment; ability to integrate recovery principles into care.
Excellent interpersonal skills for potential client engagement; ability to identify possible individualized recovery supports for clients; ability to write reports; ability to effectively present information and respond to questions; ability to communicate fluently in a second language is viewed favorably; proficient with electronic medical record.
Physical RequirementsProlonged periods of time sitting at a desk, using the phone, and working on a computer. Must be able to lift up to 20 pounds at times.
Valid Driver’s license required.

- Together with the agency medical providers, assess clients’ medical needs including withdrawal symptoms and medication side effects.
- Assists with the Suboxone induction process by taking vital signs and recording all information in the medical record, complete tox screens and gather previous labs and physical.
- Document ongoing client progress in areas that involve relevant side effects of medication.
- Assist physician in submitting, routing and ensuring completion of lab work as ordered.
- Provide face to face clinical assessments and medication education to client/family.
- Demonstrate competency in age related or specialty issues and developmental needs for each population served.
- Work with prescriber and family regarding referrals to other medical and treatment resources.
- Coordinate and schedule MAT physicals as assigned, participate in weekly case reviews and supervision.
- Maintain up-to-date case records for each client.
- Administer Vivitrol or Sublocade shots to clients with diagnosed OUD.
