Opened in 1977, Torrington Community Hospital is a critical access hospital that provides excellent medical care to residents in southeast Wyoming and bordering Nebraska communities.

We have a strong Therapies team with a lot of longevity along with cohesion to promote a great work/life balance. Evaluations and treatments are provided on a 1:1 basis, with an average of 8 patients per day with 25 beds. This allows for documentation to be completed during the workday.

As the Master Social Worker, Ambulatory Care, you will see patients in the multi-specialty clinic setting. You will assist patients and families to identify resources in the community. The top candidate will have the ability to support whole patient care/integrated care. They will assist patients and family with understanding the disease process, helping patients and families identify supports systems, and assist the patient with resources to make informed decisions about their healthcare.

The selected candidate will work alongside a team of nurses, and therapist to help the patient utilize community resources.

This is a Full-time, Monday-Friday 8:00am-5:00pm position. Community Hospital in Torrington, Wyo. is a 25-bed critical access hospital. Position Summary

This position develops, coordinates and provides social work services to patients and families who are faced with social, emotional and situational stressors precipitated by illness, injury, and/or disability. The goal is to aid in adaptation and empower the patient and the family to participate to the fullest of their abilities in the discharge planning process. This position provides developmentally appropriate care for the population that it serves which includes planning for the safe discharge and continuity of care, the ability to recognize and plan for the unique needs of all ages as well as the physically disabled, mentally ill, chronically ill and terminally ill patient.
