Clinical Case Manager – At Nexus-Woodbourne, we provide children and youth (ages 0‑21) with severe emotional and behavioral problems with safe, specially equipped foster homes that have the skills and resources to meet their complex needs.
Our Treatment Foster Care families are trained and certified in medication management, crisis intervention, and trauma‑informed care, and they’re focused on helping foster youth build healthy relationships and develop valuable life skills.
Position SummaryThe Clinical Case Manager provides assessment and treatment services to individuals, families and groups and coordinates ongoing therapeutic services and community support needed to ensure clients receive stabilization care and resources for daily living activities.

- Client service consultation
- Assists clients in linking to services, identifying barriers, and connecting with supports prior to discharge.
- Provides consultation and support to families, youth, and adults in need of services.
- Works closely with teams to create stabilization and discharge plans, develops treatment plans and utilizes a variety of treatment approaches with clients.
- Screens direct referrals, assists in triage, provides clinical assessments and mental‑health therapy; leverages organizational tools to manage multiple client needs.
- Provides assistance to families regarding resources available and serves as a resource for crisis intervention.
- Communicates with families, legal systems, schools, mental‑health providers, probation, and social‑service agencies; works with outside organizations to implement plans for adults and children entering crisis services.
- Collaborates with the treatment team for service delivery; provides timely communication to supervisory staff and team members.
- Maintains accurate paperwork and documentation required by the organization and governing agencies; keeps current files on all individuals enrolled in services.
- Produces informal and formal reports as requested by agencies, parents, referral agencies, courts, etc.
- Facilitates discharge planning meetings and develops a discharge plan that meets individual needs.
- Completes the after‑care process of discharge planning, follows up on connections to outside services, and conducts satisfaction services for providers and recipients.
- Builds and maintains relationships with adults, families, youth, and outside organizations; serves as a role model, advocates for needs, demonstrates clear, professional, respectful, strengths‑based communication.
- Maintains a database of community resources and contacts for outside providers; reports, collects and assesses data on services provided, linkages, and additional outcomes.
- Creates aftercare plans, follows up, and manages multiple priorities with effective time management.
- Participates in staff and supervision meetings and required in‑service meetings.
- Demonstrates professional, ethical conduct, integrates the Social Work Code of Ethics, maintains confidentiality, and practices self‑awareness and boundary maintenance.
- Respects diversity in cultural, social, economic, and religious backgrounds; demonstrates acceptance of differences in gender, race, age, culture, ethnicity, class, religion, and disability.
- Exhibits professionalism, accountability, cooperation among team members, and assists co‑workers as needed.
