We are hiring an experienced RN to provide in-home clinical assessments and ongoing skilled oversight for adult and geriatric clients.
This role is structured for nurses who are confident making clinical decisions, managing documentation, and working independently in the field.
If you prefer focused patient care over constant facility demands, this position offers flexibility without sacrificing professional standards.
Responsibilities- Conduct comprehensive in-home assessments
- Develop and update individualized care plans
- Monitor changes in condition and adjust care accordingly
- Complete accurate, compliant documentation
- Communicate clearly with caregivers, families, and leadership
- Ensure continuity and quality of care
This is one-on-one nursing — organized, accountable, and clinically driven.
