Frederick Health Medical Group (FHMG), a highly respected 160+ provider, multi-specialty medical group, is seeking an experienced Nurse Practitioner or Physician Assistant to join our Endocrinology team in a unique Full-Time role. This position is ideal for an APP who thrives in a collaborative environment, values high-quality patient care, and enjoys supporting a physician-led team while maintaining a balanced and rewarding clinical practice.
In this role, you will spend approximately 50% of your time providing direct patient care and 50% supporting the provider team through clinical coordination and workflow support. Unlike traditional Endocrinology roles, you’ll co-manage a shared patient panel with endocrinologists and fellow APPs, deliver evidence-based care, and help drive quality, access, and patient experience through a structured APP Team Care Model.

About the Role
As an Advanced Practice Provider at FHMG, you will function as a clinician, consultant, educator, and collaborative partner to physician colleagues. You will evaluate and manage patients with diabetes and complex endocrine disorders in an ambulatory setting, leveraging standardized protocols, collaborative care planning, and close partnership with endocrinology physicians and the broader interdisciplinary team.
Key Responsibilities
- Provide outpatient evaluation and management for patients with diabetes and a range of endocrine conditions.
- Co-manage a shared patient panel with endocrinologists in an APP Team Care Model, using standardized protocols and clear escalation pathways.
- Initiate and titrate endocrine therapies (including insulin and other diabetes medications), interpret labs/diagnostics, and adjust treatment plans.
- Interpret CGM and glucose data, address complications risk, and coordinate needed screening and follow-up.
- Educate and coach patients and families on disease management, lifestyle changes, and medication adherence.
- Coordinate care with interdisciplinary partners (nursing, diabetes education, nutrition, pharmacy, primary care) and support referrals and transitions of care.
- Document high-quality, compliant clinical notes in the EMR and support clinic access, throughput, and patient experience.
- Participate in quality improvement initiatives to improve outcomes (e.g., A1c optimization, care-gap closure, workflow standardization).
Required Qualifications
Education & Licensure
- Nurse Practitioner:
Master’s degree from an accredited nursing program - Physician Assistant:
Graduation from an accredited PA program - Active Maryland license (or ability to obtain)
- Current DEA license and Maryland Controlled Dangerous Substance (CDS) license
- Current BLS certification (American Heart Association or approved provider)
- Unrestricted license with no history of suspension or revocation
- Eligible for participation with all payers
Skills & Experience
- Prior experience in Endocrinology, Diabetes, Primary Care with strong diabetes management, or related specialty
- Strong clinical judgment and commitment to evidence-based practice
- Proficiency with electronic medical records (EMR experience preferred)
- Excellent organizational, time-management, and multitasking abilities
- Strong interpersonal, communication, and problem-solving skills
- Ability to work independently while thriving in a team-based environment
