Stella Maris is looking for Home Health Registered Nurses (RN) to join their growing Home Health team. This is an exciting opportunity to work alongside dedicated clinicians to meet the needs of clients in the community we serve.

DESCRIPTION: Under the direction of the clinical manager, the Home Health Registered Nurse performs primary nursing care functions in conformance with professional standards of home health nursing practice.

Providing individualized care to patients in their home under the plan of care approved by the primary physician. The home health nurse coordinates all services provided for each assigned patient and family.

The Home Health Registered Nurse provides leadership and advocacy in the coordination of patient care with multiple clinicians, modalities, and community resources. They manage clinical resources and transition planning for patients within the assigned caseload from pre-admission through post-discharge, actively working to provide excellent medical care.

This position requires daily travel in assigned region

QUALIFICATIONS:

• RN licensure in the State of Maryland required
• Prior experience in hospice or home health required
• OASIS Documentation experience preferred
• Valid driver's license and reliable transportation

USD $38.05/Hr. – lower to mid-portion of the range; offers above that level are reserved for candidates with advanced or highly specialized qualifications.

Max USD $57.05/Hr.
