Acts is currently seeking qualified candidates for the role of Licensed Practical Nurse (LPN) in our skilled nursing neighborhood.
In this role, you will be responsible for providing prescribed medical treatment and personal care services to our residents in skilled nursing, supervising nursing assistants and coordinating resident care according to the care plans.

Pay Range $25.60 - $40.60 / hour. Starting rate will vary based on skills and experience.

Competitive Salary $31.25- $36.00/hr and Shift Differentials!
Responsibilities- Under the direct supervision of the RN, assist in care planning and treatments for residents and consult with other nursing staff as appropriate with applying knowledge and resources
- Demonstrate basic knowledge and skills required to care for the resident population on an assigned unit
- Review resident medical records to ensure nursing practice provided is within State and Federal guidelines
- Document care provided and report changes in resident status to shift supervisor
- Review all referrals for consultation, treatment or resolution of problems are valid and applicable to the documented nurse plans
- Seek opportunities for professional growth and expansion of responsibilities to maintain competency levels
- Set and implement goals and policies consistent with the philosophy and purpose of Future Care's nursing facility
- Must hold an active LPN license issued in the state of Maryland
- 1 year of nursing experience preferred
- Previous experience caring for residents in an acute or long‑term health care facility a plus
