About the Position
White Oak Medical Center seeks to hire an experienced Care Navigator Concierge for our Outpatient Care Navigation area who will embrace our mission to extend God’s care through the ministry of physical, mental, and spiritual healing.
Responsibilities- Schedule appointments for patients and families in accordance with discharge instructions.
- Maintain awareness of local physician practices and manner in which to schedule hospital follow-up appointments.
- Notify manager when experiencing challenges to booking hospital follow-up appointments.
- Notify other members of the Patient Navigation team when patients require additional support to see physician post discharge.
- Complete required documentation in a timely and accurate manner.

- High School Diploma
- Strong customer services skills and 1-2 years’ experience working in a medical setting
- Active American Heart Association Basic Life Support (BLS) certification required

Pay Range: $18.80 - $24.18
