LMSW Case Manager

Kaiser Permanente seeks an LMSW Case Manager for our Healthcare & Medical Services department. In this role, you'll provide psychosocial assessments, discharge planning, and care coordination for diverse patient populations. You'll collaborate with physicians, nurses, and community partners to develop and implement individualized care plans, ensure smooth care transitions, and connect patients with essential resources. You'll maintain accurate documentation, support quality and utilization initiatives, and advocate for patients in line with our culture of innovation, collaboration, and continuous improvement.
