Role OverviewWe’re seeking Primary Care NPs to provide on-site care to Assisted Living/RCFE residents across Stockton, Modesto, and Amador.
What You’ll Do- Routine & urgent assessments; chronic disease management
- Post-discharge follow-ups, med reconciliation, labs/diagnostics
- Coordinate with facility staff, specialists, families/POAs
- Same-day documentation; CMS/regulatory compliance
Part-time position with a structured weekday schedule. You'll spend 3 days per week delivering care, with some days on-site across Stockton, Modesto, and Amador (strategically clustered routes designed to minimize travel and maximize time with patients) and other days via telehealth.
Qualifications- Active CA NP & RN licenses; active furnishing license; NPI, DEA, BLS
- Geriatrics or facility-based experience preferred; EMR proficiency
- Annual compensation range: $115,000–$130,000 (based on experience, schedule, and patient volume)
- Additional earning opportunities available based on coverage and panel needs
- W-2 with mileage reimbursed per policy
- Final terms outlined in the employment agreement
- On-site care with clustered routes to minimize travel
- 24/7 concierge support for scheduling, coordination, labs/orders, and pharmacy
- Modern tools that reduce admin and support same-day documentation and compliance
