Position: Staff Pharmacist - Long Term Care, Full Time

Job Overview

The Long-Term Care Staff Pharmacist plays a vital role of the Long-Term Care Pharmacy. This position requires dedication and commitment to the success and culture of the operation. This position is key in building and maintaining positive relationships with each of our clients/facilities. An understanding of the Home Town Pharmacy Mission is both essential and valuable for this role. HTP is built on the commitment of serving others –patient, community, teammate and company.

This job entails leadership, commitment, dedication, communication, empathy, and the overall understanding of both being a pharmacist and the operations of a Long-Term Care Pharmacy. A very important skill required is Communication—Listening, Verbal and Written. Responsible delegation of work is also a vital key to the success of this role as is always leading by example and nurturing/maintaining the Home Town culture.

Dispensing

Pharmacist Job Expectations

Dispensing Pharmacist job expectations are as follows:

  • Be the drug information expert in the pharmacy: answer staff and facility questions, make sure orders are clear and clinically appropriate, release entered orders from workflow performing all necessary interaction, allergy, and duplicate therapy monitoring and ensure all orders are entered correctly.
  • Responsible that all pharmacy laws are being followed at all times.
  • Check all packaged medications for proper medication, strength, lot and expiration date information for accuracy before finalizing for delivery.
  • Perform clinical calculations as required- creatinine clearance dosing recommendations, IV dosing and monitoring, warfarin dosing, and other specialty requests.
  • Monitor and help implement formulary interchanges including promote the therapeutic interchange to new providers and/or facilities.
  • Promote and maintain professional attitude and performance from all staff members.
  • Perform Medication Regimen Reviews for patient acute change or fall requests.
  • Oversee all compounding and IV admixture to ensure proper product strength and appropriate technique, as well as maintain accreditation/regulatory requirements and competencies.
  • Oversee all IV prescriptions for proper stability, dosing interval, flow rate, and dispensed supplies.
  • Conduct Comprehensive Medication Reviews and other MTM activities effectively.
  • Be responsible that the deliveries are not sent until all new orders have been processed and meds are being sent.
  • Be in the on-call rotation for after-hours coverage of facility needs.
Traveling Consulting Job Expectations (if applicable)
  • Thorough knowledge of the Home Town LTC Consultant Pharmacist Manual.
  • Thorough knowledge of Rxpertise consulting software.
  • Thorough knowledge of all Guidance to Surveyors F-tag regulations concerning pharmacy services.
  • Be available for facility Quality Assurance, Behavioral Management, Pain Management or other necessary committee meetings.
  • Meet with the Director of Nursing at each facility visit to review general clinical information, and specific concerns.
  • Perform chart reviews at an average rate of 60 charts per 8 hours after 3-4 months in a facility.
  • Print out reports with proper breakouts (nursing, medical director etc.) and present to facility within one week after consult is completed.
  • Be available via phone, or in person, if requested during facility surveys.
  • Act as a Home Town Pharmacy LTC service representative while in each facility, inquiring on the level of satisfaction with the entire scope of pharmacy services.
  • Become a member of ASCP (American Society of Consultant Pharmacy and stay current on all clinical developments.
  • Work with pharmacy school clinical students, train new consultants.
  • Build relationships with physicians and other prescribers to ensure that each patient receives optimal drug therapy. Work with prescribers to ensure support of HTP Therapeutic Interchange Program.
Job Requirements and Qualifications Education, Certificate, and Licensure
  • Bachelor’s degree or higher from an accredited college or university in Pharmacy.
  • Current Michigan Pharmacist License.
  • Current Michigan Immunization Certification.
  • Current CPR Certification.
  • GCP certified, strongly recommended.
Requirements
  • Must demonstrate practical knowledge of computers, software, and products applicable to the industry.
